Since 2015, my location for my Internal Medicine Primary Care practice is at Beth Israel Deaconess Health Care - Newton Centre,  714 Beacon Street, Newton, MA, 02459.  If you are an existing patient or hope to become a patient please call CENTRAL REGISTRATION at 617-754-8240 to register all your personal and insurance information.  For appointments call 617-332-1001 after you are registered.  If you need a refill please ask the pharmacy to fax a refill request to 617-332-5154.  If you need a referral please call the office so that they can assist you in this process.

 

When making an appoinment for your first visit, please release your medical records to yourself to bring them with you to your appointments or release them directly to the office.
